If it is the first time that you access the portal, you will need to request a portal activation code from Dr. Davison's office, and then proceed to set up a user ID and password. The process is simple and straightforward. However, if you have any difficulties, please contact our office.

Please Note
  • Please bring your current medical insurance card(s) to each appointment
  • Your co-pay is due at the time of your appointment
  • There is a $20 charge for each returned check
  • There is a $50 fee for any patient that does not give at least 24 hours notice for not keeping an appointment
  • A complete eye exam can take up to 90 minutes so please budget your time accordingly
